CarersAs a Practice we recognise the importance the role of carer provides. Therefore we are committed to supporting carers under many different situations.
If you care, on a regular basis for a child, relative, friend, partner or neighbour, who is unable to manage on their own please let us know so we can put this information on your record.
Its very helpful to have details of patients' carers and we will always check that you are happy to for us to put your contact details onto our computer system.
If the person you care for (aged over 16) would like you to be able to access test results and other medical information regarding their, you will need to provide a signed a letter from them. A message will then be placed on their medical record informing the staff of the agreement.
